MODIFIED PERIOSTEAL INHIBITION WITH LAMINA IN FLEXIBLE CORTICAL BONE

Inhibition of osteoclastic remodelling using an equine-derived Flex Cortical Sheet.

The clinical case concerned a patient with fractured tooth element 14, together with vestibular fenestration, extended to the apex of the tooth element, and palatal resorption of 2 mm. The operation involved incision of the papillae and full-thickness dissection of the vestibular tissues extending beyond the fenestration, with elevation of the periosteum, in order to create a space for the cortical lamina to fit. The lamina was appropriately cut to fit the housing and was hydrated with the patient’s blood. The lamina was then glued onto the vestibular side, using drops of fibrin glue. A CBCT examination was performed 4 months after surgery, which showed excellent remineralisation of the alveolus, with complete maintenance of the cortical profile, complete closure of the fenestration and no bone resorption. A 4 x 10 mm implant was then placed in the regenerated site. After a further 3 months, the implant was prosthetised with a Curvomax abutment and a cemented metal ceramic crown. The subsequent check-up showed a perfect integration of the prosthesis and an optimal aesthetic result, with restoration of the physiological vestibular convexity.
