A fenestration in a post-extractive socket can be managed by using a bone paste substitute protected by a suitable membrane.

The sheet reported the case of a patient with coronal fractures involving teeth 1.4 and 1.5 with the relevant clinical crowns below the bone crest and rampant caries below the 1.3 bone margins. After the usual antibiotic prophylaxis, teeth 1.3-1.4-1.5 were extracted, and two osseointegrated implants were inserted at the same time in position 1.4 and 1.5. The post-extractive site in position 1.3, instead, presented with a large vestibular dehiscence, not visible on X-ray. A regenerative intervention was therefore performed, by means of an ultimate generation bone paste obtained through an enzymatic deantigenation process, using a pericardium membrane to cover the bone graft and completing the whole with non-resorbable sutures.
