The present report describes a case of a 38-year old patient who had his radius fractured because of a car accident. A first surgery involved stabilizing the reduced fracture with a plaque, but bone did not heal as an infection developed. Three months later, a second surgery followed involving placing an antibiotic-filled spacer and, as the infection was not cured, the spacer was replaced three months later, adding a second antibiotic. The patient initiated also an antibiotic therapy per os. Six month later, the patients was grafted with a vascularized fibula graft. Yet, the graft did not integrate and a nonunion developed. A year later the nonunion was treated by grafting autogenous bone from the iliac crest, an equine bone substitute and equine DBM added with autologous BMC and platelet rich plasms (PRP). At the 6-month control the bone structure appeared successfully reconstructed.
