This article presents two clinical cases of immediate postextractive implant placement on lower posterior molars associated with regenerative surgery using heterologous preserved collagen graft of equine origin (Osteoxenon cortical-cancellous granules, Bioteck Spa). The first clinical case deals with a 73-year-old woman who presented with the loss of the crown of 36 and a complete fracture of the residual tooth element at the level of the pulpal floor in which a postextractive implant was placed by exploiting the interradicular septum. The second clinical case, on the other hand, deals with a 65-year-old man with a coronal-radicular fracture of 46. Since it was also impossible in this case to retain the original tooth element, it was decided in favor of surgery with immediate postextractive implantation.
