The 76-year-old patient presented to clinical observation because of a vestibular fistula in tooth element 36. The endoral radiography showed the presence of a screw in the distal root of the tooth, a possible cause of the root fracture. The tooth element was considered unrecoverable. At 3 months after extraction, there was evidence of complete soft tissue healing but on CBCT examination, however, there was a marked reduction in bone thickness in the area affected by the extraction, necessitating a Guided Bone Regeneration (GBR) procedure performed at the same time as implant placement. Therefore, 2 implants were placed and then heat-treated slow-resorbing equine granules were placed. A resorbable equine pericardium membrane was affixed to protect the bone graft. CBCT examination performed at 6 months showed complete peri-implant bone regeneration with restoration of adequate vestibular bone thickness. At 6 months after GBR, 2 monolithic zirconium crowns were screwed in, which ensured complete and effective final prosthetic rehabilitation.
