The clinical case concerned a patient with vestibular fistula and pain in tooth element 14, of which slight mobility was noted. Analysis by CBCT confirmed the suspicion of a vertical fracture of the element in question and made clear both the large distal periodontal defect and the small amount of alveolar bone. After an appropriate anamnestic evaluation, the decision was made to remove the compromised tooth, with atraumatic root extraction; it was also decided for an alveolar preservation to be performed by inserting a 0.5-mm-thick flexible cortical lamina packed at vestibular level between bone and periosteum. The CBCT at 4 months after the operation showed good maintenance of the pre-extraction volumes, with adequate mineralisation of the alveolus. It was therefore possible to proceed with implant placement. At 10 months after bone regeneration, the definitive prosthesis was delivered. An optimal aesthetic result could be appreciated, with restoration of the physiological vestibular convexity.
