Laser therapy and biomaterials in the treatment of periodontal pockets: case report

Nonsurgical periodontal instrumentation is necessary as initial treatment, and as maintenance therapy, of any plaque-induced inflammatory pathology, after proper diagnosis, which remains absolutely crucial, unique and unequivocal. The goal of periodontal therapy is to maintain natural dentition, or any dental implants, for as long as possible, preserving or improving oral health, chewing function, phonation as well as aesthetics, while also preserving supporting tissues. The use of lasers in the treatment of periodontal lesions is still the subject of heated controversy. In the consensus document, Best Evidence Consensus (BEC), experts convened by the American Academy of Periodontology (AAP) conclude that there is insufficient scientific evidence from the international literature for laser-assisted periodontal treatment. Based on recent human histological studies, however, there is evidence of periodontal tissue regeneration after laser-assisted treatment, both on natural teeth and around implants. There is also clinical evidence of bone remineralization, which is possible but certainly not predictable, after nonsurgical laser-assisted periodontal treatment. In addition to laser-assisted periodontal instrumentation, there are several biomaterials on the market to be applied after mechanical debridement, which find clinical indications as additional strategies to manage any clinical instability with a nonsurgical approach. In case of recurrence of infection, with increased pocket depth, the surgical approach is indicated, particularly in the presence of periodontal pockets ≥6 mm. The purpose of this article is to illustrate a laser-assisted periodontal therapy protocol associated with the use of a novel collagen hydrogel, documenting the clinical and radiographic results, to successfully manage, at least in the short term, clinical instability with a nonsurgical approach.
