A combined approach using a heterologous wedge with autologous stem cells in an advanced case of knee osteoarthritis
Today we present a case of conservative surgery for knee osteoarthritis performed by Dr. Paolo Filippini (Perugia, Italy), which involved a tibial osteotomy combined with a heterologous bone wedge and the use of autologous stem cells.
The 35-year-old patient, who had already undergone extensor realignment for a patellar dislocation, continued to experience pain and functional limitations despite a cartilage transplant and two arthroscopies. The clinical picture showed a deficit in extension (10°), reduced flexion (80°), and involvement of the internal femoral-tibial compartment with varus.
Given the patient’s age, further arthroscopic debridement combined with the use of autologous stem cells was performed. At the same time, an additional tibial osteotomy was performed. A cancellous wedge of equine origin (Osteoplant® Cancellous Wedge) was used to fill the osteotomy space, promoting bone regeneration.
Osteoplant® bone grafts undergo the exclusive Zymo-Teck® process, which preserves both the mineral component and the bone collagen in its native form. The presence of collagen ensures the physiological remodeling of the graft with the patient’s new vital bone, as well as providing excellent mechanical properties that allow the graft to be processed without the risk of fracture.
This case highlights how a prosthesis is not always the only solution: integrated conservative strategies can alleviate pain and improve movement even in advanced osteoarthritis.
